In this week's newsletter, we would be touching on the topic regarding dermal fillers which is an injectable fillers into the skin to help in smoothening the aged skin to restore one's youthful appearance. There consist of natural and synthetic fillers, in which natural fillers usually contain hyaluronic acid which can last between 6-12months. Whereas synthetic fillers include poly-L-lactic acid (helps to stimulate collagen production) and calcium hydroxyapatite that last up to 1-2years.

Top 5 Facial Injection Danger Zones
1. Glabella (Between the eyebrows and just above the nose)
Recommended injection point into offending rhytides with a Low-G filler using a serial puncture technique. When injecting in the brow area, digital pressure should be used to occlude supraorbital and supratrochlear vessels along the rim to prevent back flow in the event of inadvertent injection of vasculature.
2. Temple area injectables
To maximise safety in this area, we generally mix filler in 1:1 ratio with 0.5% lidocaine with epinephrine. Starting at the pretrichial line, the injection proceeds lateral to medial in the superficial subcutaneous plane just below the dermis.
3. Nasolabial grooves (Lines between corner of nose and the mouth)
When injecting into the nasolabial grooves, use of cannula should be considered, given the close proximity to the facial artery and its potential location within the subcutaneous tissue. Injection into the deep dermis or superficial subcutaneous tissue should be performed un the inferior two-thirds with anterograde and retrograde linear thread technique.
4. Infraorbital region
Overall, when injecting deep into the mid-face, measurements should be kept in mind to avoid intravascular cannulation or vascular injury. Avoid direct deep injections into this area, going just lateral. Injections more medial, especially approaching the medial cants, should be avoided completely.
5. Nasal Augmentation
When injection in the nose, injections should be carried out deep to musculoaponeurotic layers in the preperichondrial and preperiosteal layers to avoid injury or cannulation of vessels. The vasculature in the nose is superficially located beneath dermis.
