Lateral Canthoplasty as Access and Aesthetic Tool: A 4-Step Technique for the Young Asian Lower Lid

A technique-focused review of combined lateral canthoplasty and transconjunctival lower blepharoplasty (Kim JY et al., Archives of Aesthetic Plastic Surgery, 2014;20(3):155–159)

The Clinical Problem Lower eyelid bags are usually framed as an aging concern — but in young Asian patients, congenital infraorbital fat prolapse frequently coexists with a short palpebral fissure and an up-slanting lateral canthal angle. These patients present with a distinct and dual request: correct the tired, bulging lower lid and create larger, brighter, oval-shaped eyes — all without a visible scar. The anatomy compounds the challenge. The orbital septum is divided into an upper reinforced portion, supported by the capsulopalpebral fascia, and a lower unreinforced portion that lacks this support. In Asian patients, the unreinforced zone is proportionally larger and the lower lid thicker than in Western patients — which helps explain why fat herniation appears early, often congenitally, in the absence of any tissue laxity. The conventional transconjunctival approach solves the scar problem but creates an exposure problem: a narrow operative field in which accurate fat assessment and septal manipulation are genuinely difficult. Suturing the septum to the often-deficient periosteum of the lateral orbital rim is particularly frustrating in patients with small orbits — precisely the population in question.


Kim and colleagues at Konkuk University Medical Center offer an elegant solution: make the lateral canthotomy incision do double duty. First it serves as surgical access, converting a keyhole transconjunctival procedure into an open-field dissection. Then the same incision becomes the vehicle for a cosmetic lateral canthoplasty. One incision, two objectives.

Their series — 59 young Asian patients (58 women, 1 man; mean age 26.9 years, range 19–37; operated June 2011 to May 2014) — demonstrates the approach is safe, reproducible, and delivers measurable fissure lengthening. Patient Selection Ideal candidates:

  • Visible lower eyelid bulging without excess skin laxity

  • Desire to correct an up-slanting lateral canthal angle or short lateral palpebral fissure

Absolute exclusions:

  • Dry eye

  • Severe exophthalmos

  • Patients requesting an S-shaped lower lid margin

These exclusions are biomechanical, not arbitrary. Canthal repositioning alters lid-globe apposition and tear film dynamics; in a dry or proptotic eye, lowering and lengthening the lateral canthus invites exposure symptoms and lid malposition that no amount of technical precision will prevent.

The 4-Step Technique in Depth Step 1: Marking, Anesthesia, and the Inferolateral Canthotomy


Preoperative marking is done with the patient sitting upright — a non-negotiable detail. Supine positioning allows orbital fat to fall posteriorly and reliably underestimates the true projection of the bulge. Two things are marked: a 7–8 mm line for the inferolateral canthotomy, extending from the lateral canthal angle in an inferolateral direction, and the prominence of the inferior orbital fat pads.

Anesthesia is fully local: lidocaine with 1:100,000 epinephrine infiltrated diffusely into the lower periorbital tissues, the conjunctivae, and the lateral canthal area. Topical anesthetic drops anesthetize the ocular surface, and corneal protectors are inserted before any incision.

Traction sutures are then placed in the lower eyelid, and the canthotomy opens the case. The lateral palpebral fissure is incised inferolaterally through all three layers — skin, orbicularis oculi, and conjunctiva. The angle of this cut matters: the inferolateral vector is what will later permit both horizontal lengthening of the fissure and lowering of the up-slanted canthal angle, rather than lengthening alone.

The pivotal maneuver of the entire operation follows: the superficial fanning fibers of the lateral canthal tendon are deliberately transected. Once these fibers are released, the lower eyelid can be freely everted on its traction sutures. This single move is what transforms the transconjunctival approach — the lid is no longer a curtain you work behind, but a flap you reflect out of the way. Every subsequent step benefits from the exposure created here.


Step 2: Transconjunctival Dissection in the Preseptal Plane With the lid everted, the transconjunctival incision is placed 3–4 mm below the lid margin, at the inferior edge of the tarsal plate. Placement here — rather than lower in the fornix — preserves the tarsus intact while entering directly into the preseptal plane. The incision runs in a mediolateral direction and is extended laterally as far as necessary; because it will merge with the canthotomy, lateral extension costs nothing in visibility or scarring. Dissection proceeds with blunt scissor spreading along the preseptal plane — between orbicularis anteriorly and the orbital septum posteriorly — carried inferiorly to just above the arcus marginalis. Two discipline points define this step:

  1. Keep the orbital septum intact during the descent. Premature septal violation lets fat herniate into the field, obscures the anatomy, and destroys the very structure you intend to reset in Step 3.

  2. Stop at the arcus marginalis. This is the landmark where the septum inserts onto the orbital rim, and it is where the septum will be opened — under direct vision, not blindly.


The reward of Steps 1 and 2 combined is a wide, bloodless preseptal pocket with the entire septum, from tarsus to rim, laid out in front of you — exposure that a standard transconjunctival approach simply cannot offer.

Step 3: Conservative Fat Excision and the Orbital Septal Reset The septum is now opened at the arcus marginalis using electrocautery. The three fat compartments come into view: the medial and central pads, separated by the inferior oblique muscle (which must be identified and protected), and the lateral pad, demarcated from the central pad by the arcuate expansion of Lockwood's ligament.

Excess fat is excised from all three pads — conservatively. The authors are explicit on this point: aggressive excision in young Asian lids risks local hollowing and a sunken, skeletonized appearance that is far harder to correct than residual fullness. Fat removal here is a debulking adjunct, not the definitive correction.

The definitive correction is the orbital septal reset, the structural heart of the operation. The lower edge of the strong, reinforced upper portion of the orbital septum is advanced and sutured to the periosteum of the orbital rim or arcus marginalis with a running 5-0 absorbable suture, beginning medially and working laterally. The logic is anatomic: the weak, unreinforced lower septum — the segment that allowed herniation in the first place — is effectively bypassed, and the diaphragm function of the septum is rebuilt using its strongest available tissue anchored to fixed bone-periosteum.

The result is a flat, load-bearing anterior wall over the orbital fat. This is why the series recorded zero recurrences: the reset addresses the mechanism of the bulge, whereas fat excision alone addresses only its volume. It simultaneously guards against undercorrection and against the recurrence that follows purely subtractive techniques. The conjunctiva is then closed medial to lateral with a running 6-0 absorbable suture with buried knots, keeping suture material off the ocular surface. Step 4: Tarsal-Periosteal Canthoplasty and Layered Canthal Reconstruction

The same canthotomy incision now converts to its aesthetic role.

A mattress suture of 5-0 non-absorbable material is passed through the tarsal plate, the orbicularis muscle, and the conjunctiva of the lower lid, then anchored to the periosteum on the inner aspect of the lateral orbital rim and tightened until the canthus sits at the desired position. Rigid tarsal-periosteal fixation is the point: it resists relapse and web formation, and it gives the surgeon a titratable control — tension and anchor position determine, in one stitch, the degree of horizontal lengthening and the degree of lowering of the canthal angle, balanced against flattening of the lower lid margin.

The critical caveat — and the paper's most valuable lesson — concerns the anteroposterior position of that anchor. All five complications in the series (three entropions, two rounded canthal angles) traced to fixation placed too far on the inner (posterior) surface of the rim, generating an excessive posterior vector on the lid margin. All resolved after revision with fixation moved to the anterior surface of the orbital rim, with no further complications. Internalize this before your first case: the fixation vector, not the fixation concept, is where this operation goes wrong.

Target geometry: the lateral canthus is set 1–2 mm higher than the medial canthus — preserving a subtle positive canthal tilt even after lowering the exaggerated up-slant.

Closure is deliberately layered to prevent the two classic stigmata of lateral canthal surgery:

  • A small wedge resection of lower lid skin eliminates the dog-ear that redundant skin would otherwise form at the new, longer fissure. This substitutes for the subciliary incision used in earlier techniques (Jang et al., 2009) — avoiding both the visible lid scar and denervation of the orbicularis, and with them the risk of postoperative lid retraction or ectropion.

  • Webbing is prevented by asymmetric layered closure of the new canthal angle: at the upper lid, palpebral conjunctiva is sutured to skin; at the lower lid, skin is sutured to skin. This reconstructs a crisp mucocutaneous junction at the reformed angle rather than a blunted fold.

Chemosis was mostly self-limited; select cases required lubricants and steroid drops. Lateral canthal scars were cosmetically acceptable by two months. Most patients reported satisfactory aesthetic results at follow-up.


That the entire complication profile maps to a single, correctable technical variable — fixation depth on the rim — is arguably the study's most practically useful finding.


Limitations to Weigh Before Adopting Short follow-up (mean 3.4 months; minimum 2 months): late recurrence, long-term canthal stability, and mature scar appearance remain unassessed.

  • Retrospective, single-center, no comparator arm: "most patients satisfied" is not a validated patient-reported outcome measure.

  • Homogeneous cohort (98% female, ages 19–37): generalizability to older patients or lids with skin laxity is untested — and by design excluded.

  • Objective outcomes limited to photographic fissure-length measurement.

None of these undermine the technical argument, but they should temper durability claims when counseling patients.

In Conclusion For young Asian patients with congenital lower lid fat prolapse plus a short, up-slanted palpebral fissure, this single-incision combination delivers:

  1. Open-field exposure for septal work without any visible lid incision — the canthotomy is the retractor

  2. A structural rather than purely subtractive correction — the septal reset explains the zero recurrence rate

  3. Simultaneous canthal reshaping averaging nearly 3 mm of fissure lengthening with a controlled, titratable canthal angle

  4. One well-defined failure mode with a known fix: keep the tarsal-periosteal fixation on the anterior surface of the lateral orbital rim

Respect the exclusion criteria, be conservative with fat, and get the fixation vector right — and this technique converts two operations' worth of patient goals into one incision's worth of morbidity.
